We Need The Following From You: |

Actual passport, signed, valid at least 6 month beyond stay
with blank visa page
US passport page 22, 23 and 24 are not visa pages
and won't be acceptable for visas. 2 Two visa application form per person, completely filled-out and signed 3 Two recent passport-type photograph (please glue or staple the photo to the application form) 4 Copy of round trip ticket, or itinerary showing flight information and dates of departure to and from Brazil. 5 Proof of residence in the Consular Jurisdiction (eg. Copy of drivers license, Utility bill etc) 6 FBI clearance issued within the last 3 months, certifying absence of criminal record. 7 Approval of contract by Brazil Ministry of Labor or regional Brazil Department of Labor arranged by your Brazil Sponsor required. Processing of visa cannot commence until approval has been faxed to the Consulate General of Brazil in New York. 8 Please complete and print the Visa Service Request Form  |
