China Visa Application Forms

• Please fill out the application form on your computer, print a hard copy and sign it on section 6.2.
• Hand written form is not acceptable.
• Please check for omission and error before you print the form.
• All fields in are mandatory. If a section doesn't apply, please type in "None" or "N/A'. Incomplete form will be rejected.
• Change or correction is not allowed on the printed form. You must print a new form in case of any change or correction.
• We strongly suggest you to save the completed form on your computer in case of any change (choose File, Save from menu)
• Please refer to V.2011A Instruction  for more instruction and hint.
• Non-US passport holders must also fill out V.2011B form.
