2  Copy of your U.S. Green Card or valid U.S. visa
3  One visa application form per person, completely filled-out and signed. Click for form instruction
4  Two recent passport-type photograph (please glue or staple the photo to the application form)
5  Please complete and print the Visa Service Request Form
