| Applicant who plans to go to China for marriage registration shall apply for a Single Status Affidavit Authentication, also known as single/non-marital status certificate. | ||
| Important Notice: Local marriage offices in China may have special preferences in the way you fill out your affidavit. Please check with the respective marriage office in China for details. We strongly recommend you send a draft copy to China to check for acceptability before you submit your application to us. Affidavit of Marital Status Form Download and fill out the pre-formatted Affidavit of Marital Status Fill out 2 copies: one copy completed, signed and notarized; the other copy completed and signed only. Both copy needs to be original, no photocopy. Please make sure name on this form match exactly name on your passport. |

Authentication Application Form Download and fill out Authentication Application Form List content of document as: "Affidavit of Marital Status" only |

Copy of Your Passport Photocopy the the page that has your name, photo and signature. |

Copy of Divorce Decree (if any) Photocopy only. Do not send the original, it won't be returned. |
