China Crew Member Visa - C Visa

The validity of Crew Member Visa varies depending on the agreement between particular airlines or cruises and Chinese authority. The validity could be 6 months, 12 months or 24 months accordingly. The validity that applicant specified on your application form may be ignored.

Please Prepare and Send the Following Items

Actual passport, signed, valid at least 15 month validity left with blank visa page

* Please submit your current passport. The Chinese visa will be affixed into the passport, therefore your actual passport is required.

* Passport remaining validity required: 15 months for 12 months visa, 9 months for 6 months visas. If your passport has less than the required validity left, you must renew your passport first. If you need passport services, please visit our expedited passport services section.

* The last 3 pages in US passports are not visa pages. These are amendment pages, reserved by the US Department of State for amendment to the passport. They won't be acceptable for visas. If your passport has no blank visa pages left, you may add additional visa pages. If you need passport services, please visit our expedited passport services section.

* Non-US passport holders must provide photocopy of the US Green Card, or a valid US visa.

* Some non-US passport holders (holding passport from countries on the attached list ) are required to apply in person.

Complete Visa Application Form V.2011A

Please fill out the application form on your computer, print a hard copy and sign it on section 6.2.
Hand written form is not acceptable.
Please check for omission and error before you print the form.
All fields in are mandatory. If a section doesn't apply, please type in "None" or "N/A'. Incomplete form will be rejected.
Change or correction is not allowed on the printed form. You must print a new form in case of any change or correction.
We strongly suggest you to save the completed form on your computer in case of any change (choose File, Save from menu)
We also suggest you to email us your saved form for review first, if you need need Express or Express Plus Service.
You send as email attachment to: form@visarite.com, On email subject, please type: V2011form for review from your name.

Non-US passport holders must also fill out V.2011B form.

One recent passport-type photograph

Full face, frontal with a plain white or off-white background, 2"x2" in size.
Photo printed on regular paper or photo of poor quality is not acceptable.
Photo needs to be taken within 6 months. If you use the same photo as in your passport and your passport was issued more than 6 months ago, the photo will be rejcted.
Please use paper clip to attach the photo to the form. Do not staple or glue the photo!

The letters needs to be on the cruise company letter head and addressed to the Chinese Consulate.

Remarks:
Return Shipping Fee listed is for return addresses within the continental U.S. If your return address is in Alaska, Canada, Hawaii, Puerto Rico or the US Virgin Islands, please include an additional $10.00 per order.
Saturday Delivery is not included. Saturday Delivery can only be arranged upon request, additional fee will apply.

Forms of Payment:

We accept money order, cashiers check, bank check, certified check or cash. Please make check or money order payable to VisaRite Services Inc. You can write the total amount on one check or money order, no need to send separate checks or money order. Personal check is not acceptable.

You may also pay by credit card. If you select this option, please fill out and sign one credit card charge authorization form and enclose it in your application package.
