| 1 | Actual passport, signed, has sufficient*9 months validity left and one blank visa page | |
| US passport page 22, 23 and 24 are not visa pages and won't be acceptable for visas. If you also need passport service, please refer to: US passport Adding Visa Page or US Passport Renewal | ||
| * Passport Remaining Validity Required: 9 months for 6 months visas, minimum validity of 6 months required for Single Entry Visa. | ||
| Non US passport holders are required to provide photocopy of US Green Card or valid US visa. Requirement may change without notice. | ||

| Completely filled-out and signed. Fill out either on your computer or using black or blue pen. Do not use pencil or pen with red ink. | ||

| Full face, frontal with a plain white or off-white background, 2"x2" in size. Photo printed on regular paper or photo of poor quality is not acceptable. | ||
| 4 | An Official Visa Notification (also called Official Invitation)

| 5 | A business letter on your company letterhead | |
| A business letter on your company letterhead addressed to "Consulate of the Peoples Republic of China, Visa Section". The letter must specify the employee name, position, explaining the business intention and duration of the trip, company and person to visit, guaranteeing financial support for you while in China and return transportation (click for a sample business letter). Those self-employed also needs to provide photocopy of the business registration or license. | ||
| 6 | A photocopy of round trip air tickets and hotel reservation

| If you would like to apply for a double entry visa, your flight itinerary needs to show two entry is necessary for the same trip. Or your request will be ignored and a single entry will be issued instead. | ||

Return Shipping Fee: $20.00. Passports will be returned by FedEx or UPS

or cash. Please make check or money order payable to VisaRite Services Inc.
You can write the total amount on one check or money order,
no need to send separate checks or money order. Personal
check is not acceptable for Express and Express Plus Service.
You may also pay by American Express card (other credit cards are not accepted
at this moment). If you select this option, please fill out one credit
card charge authorization form and enclose it in your application package. |
