Completely filled-out and signed. Fill out either on your computer or using black or blue pen. Do not use pencil or pen with red ink. Consulate may reject your application, if section 4 is not completed. Important: section 4.1 - 4.6 must be your contact information in the US; Section 4.7 - 4.10 must be your contact information in China. Do not leave them blank. Please refer to Q-2007 Explained for detail instruction and hint. |

| Full face, frontal with a plain white or off-white background, 2"x2" in size. Photo printed on regular paper or photo of poor quality is not acceptable. Please refer to the link for detail photo specification requirement. | ||
