Please Prepare Following Documents According the Instruction: | |

Yes, 2 copies. One original
and one photocopy is fine. |

|
Completely filled-out and signed. Fill
out either on your computer or using black or blue pen. Do not use pencil
or pen with red ink. |

|
Section 1.3 asks for your Chinese name. It can not be left blank for who
with place of birth in China, Taiwan and Hong Kong. Please fill the Chinese
Characters. |
||
|
Section 1.8 asks for your Place of Birth. You need to fill in the city
and province. |
||
|
Section 2.2 asks for your Intended Number of Entries. Single or Double entry
should not be any problem for most applicant, however, if you want to apply
for multiple entry visa, please make sure you can meet
the conditions. |
||
|
Section 2.4 asks for your Longest Intended Stay in China. Please fill in
30, 60 or 90 days. 120 days may also be requested, but not guaranteed. |
||
|
Important: section
4.1 - 4.6 must be your contact information in the US; Section 4.7 - 4.10
must be your contact information in China. Do
not leave them blank. Consulate will reject
your application, if section 4 is not completed. |

| In order for us to serve all applicants efficiently, please submit the Online Visa Service Request Form and bring the confirmation page with your application. Application without the Visa Service Request Form will not be accepted. |

| 4 | One recent passport-type photograph | |
| Full face, frontal with a plain white or off-white background, 2"x2" in size. Photo printed on regular paper, photo with background color other than white or off-white, or photo of poor quality is not acceptable. Please refer to the link for detail photo requirements | ||
| 5 | Photocopy of previous Chinese visa and entry stamp | |
Make a clear photocopy of your most recent Chinese visa and entry stamp. If you have never applied for Chinese visa before; or applied for the visa, but you didn't go; or you are not able to provide a copy of previous Chinese and entry stamp, you are NOT qualified for multiple entry visa. In this case, you may only apply for a single or double entry visa. |

| 6 | Additional Requirement for Chinese Descents | |
| 1. For the first visa application of the foreign-born child of Mainland Chinese descendents, a copy child's birth certificate and a copy of parents' foreign passport or a copy of foreign permanent residence identity card (for example: the US green card) is needed along with the application. | ||
| 2. Applicant born in Mainland China and had a PRC passport, applying for the Chinese visa for the very first time, must also provide their Chinese passport along with the application. | ||
| 3. Applicant born in Hong Kong, Macao and Taiwan, applying for the Chinese visa for the very first time, must also provide a photocopy of their expired Hong Kong, Macao or Taiwan passport, or copy of government issued ID or birth certificate from Hong Kong, Macao or Taiwan. If you current name is different from the name your in your birth certificate, ID or old passport, you must provide name change documents. | ||
| 4. Applicant born in Mainland China, Hong Kong, Macao and Taiwan, applied for China visa before on a different passport, needs to provide a copy of previous Chinese visa along with the application. If you had a old version stamp visa instead of visa sticker, you also need to send old passport together. | ||
| 5. Applicant born in Hong Kong, Macao, Taiwan and mainland China must provide their name in Chinese Characters and city of birth on the China visa application form. (section 1.3 and 1.8 on Q2007 form) | ||
| 6. Applicant, who were adopted from China, applying for Chinese visa the first time, must provide original Chinese passport, a copy of the adoption certificate, a copy of the US Naturalization Certificate along with the application. If the name on the adoption certificate is different from the name on US passport, a copy of name change court order is also required. | ||
| 7. Applicant born
in Mainland China and had a PRC passport, applied for China visa before
on a different passport, needs to provide a copy of previous Chinese visa
and entry stamp from old passport. |
