Instructions for Replacing a Limited Passport

You must call for an appointment number before you mail in the application.

STEP 1 - Gather US Passport Application Materials

(1) Your Most Recent US Passport

Your most recent limited U.S. passport must be issued less than one year, or you may need to apply for a passport renewal

Your most recent limited U.S. passport was issued outside the United States and limited in validity for one year or less.

Your most recent limited U.S. passport was issued in the United States to make an emergency trip to overseas. Any documents missing at the time of the original application must be provided.

(2) Your Completed DS-5504 Form (Do not use pencil or pen with red ink.)

DS-5504 form is available at your local post office or clerk of court, or click here to download the form. Please print out and submit only one-sided pages of the application.

Some applicant may be required to submit DS-82 form instead. Please call for details.

(3) Two Identical Passport Photos

(4) Proof of Citizenship (provide 1 of the following)

Previous passport

Original or Certified birth certificate

Original Certificate of naturalization
All citizenship documents will be returned with your new passport

(5) Proof of Identity

Make a clear photocopy of driver's license with a photo on it. The license must have been issued for at least 6 months. If the above mentioned Driver's License isn't available, you will need to provide additional Proof of Identity

(6) Travel Evidence (provide 1 of the following)

Copy of your plane ticket showing departure date

Computer generated flight itinerary from the travel agent or airline

If travel is for business and neither of above is available, you may use a business letter.

(7) Letter of Authorization

These letter's must have original signatures-faxes or photo copies not accepted. Click for Letter of Authorization. Please print, fill-out and sign the letter

(8) Government Fees

Make one check payable to "U.S. Department of State" for $60 (this is the government processing and expedite fee). Be sure to include your date of birth on the check.

Forms of Payment: We accept check, money order or cash. Please make check or money order payable to VisaRite Services Inc. You can write the total amount on one check or money order, no need to send separate checks or money order. You may also pay by American Express card (other credit cards are not accepted at this moment). If you select this option, please fill out one credit card charge authorization form and enclose it in your application package.

Remarks: Return Shipping Fee listed is for return address within the continental U.S. If your return address is in Alaska, Canada, Hawaii, Puerto Rico or the US Virgin Islands, please include an additional $10.00 per order.
