Instructions for Renewal of Passport

You must call for an appointment number before you mail in the application.

STEP 1 - Prepare Passport Application Paperwork

(1) Your Most Recent Passport. (This passport will be returned with your new one)

Your passport must meet all following requirements, or you may need to apply for new passport.

The passport must be issued within the last 15 years

The passport must be issued when you were age 16 or over

You can submit your most recent US passport and your passport is undamaged

(2) Passport Application Form DS-82 (Do not use pencil or pen with red ink.)

Option 2: Fill out DS-82 form online from US State Department website

Do NOT use pencil or pen with red ink. Please print out and submit only one-sided pages of the application. This form may also be available at your local post office. Please make sure signature on this form match the signature on your passport.

Please do not select Passport Card option, because it may delay the process and additional fee will apply.

(3) Two Identical Passport Photos

(4) Court Documentation if your name has changed (Only needed when your name is changed)

Such as a marriage certificate, court order, divorce decree or adoption decree. The name change document must bear the official seal of the issuing authority. Uncertified copies or notarized documents can not be accepted. All documents will be returned to you with your passport.

(5) Travel Evidence (provide 1 of the following)

Copy of your plane ticket showing departure date

Computer generated flight itinerary from the travel agent or airline

If travel is for business and neither of above is available, you may use a business letter.

(6) Letter of Authorization

This is your authorization to VisaRite to submit your application. Click the link for Letter of Authorization. The Authorization Letter must have your original signature. Faxes or photo copies are not accepted. Please make sure signature on this letter match the signature on your passport.

(7) Government Fees

Make one check payable to "U.S. Department of State" for $135 (this is the government processing and expedite fee). Be sure to include your date of birth on the check. If you have multiple applications, please make separate check for each applicant. Do not combine them into one check. Starter check is not acceptable.

(8) VisaRite Service Fee

Forms of Payment: We accept money order, cashiers check, bank check, certified check, company check or cash. Please make check or money order payable to VisaRite Services Inc. You may also pay by credit card. If you select this option, please fill out and sign one credit card charge authorization form and enclose it in your application package.

Remarks: Return Shipping Fee listed is for return address within the continental U.S. If your return address is in Alaska, Canada, Hawaii, Puerto Rico or the US Virgin Islands, please include an additional $10.00 per order.
