1 - Gather US Passport Application Materials
(1) Your Valid Passport.
Passports can only be amended if following conditions are all met,
or you may need to apply for a passport
renewal
1. Your most recent U.S. passport was issued less than one year ago.
2. Your can submit my most recent U.S. passport.
3. You have had a change in your identifying information or your identifying
information was printed incorrectly
(2) Your Completed DS-5504 Form (Do not
use pencil or pen with red ink.)
DS-5504 form is available at your local post office or clerk of

download the form. Please print out and submit only one-sided
pages of the application.
(3) Two Identical Passport Photos

(4) Court documentation if your name has changed.
You must submit a marriage certificate, divorce decree or court
order showing the change of name. The name change document must bear
the official seal of the issuing authority. Uncertified copies or
notarized documents can not be accepted. All documents will be returned
to you with your passport.
(5) Travel Evidence (include 1 of the following)
Copy of your plane ticket showing departure date
Computer generated flight itinerary from the travel agent or airline
If travel is for business and neither of above is available, you may
use a business
letter.
(6) Letter of Authorization
These letter's must have original signatures-faxes or photo copies
not accepted. Click for Letter of
Authorization. Please print, fill-out and sign the letter
(7) Government Fees
Make one check payable to "U.S. Department of State" for
$60 (this is the government processing and expedite fee). Be sure
to include your date of birth on the check.
Starter check is not acceptable.

check, company check or cash. Please make check or money order
payable to VisaRite Services Inc. You can write the total amount
on one check or money order, no need to send
separate checks or money order.
You may also pay by credit card. If you select this option,
please fill out and sign one credit
card charge authorization form and enclose it in your application
package.

Return Shipping Fee listed is for return address within the
continental U.S. If your return address is in Alaska, Canada,
Hawaii, Puerto Rico or the US Virgin Islands, please include
an additional $10.00 per order.
